• If director and shareholder is the same person, only one (1) copy of this form needs to be completed.
• All Directors / Shareholders are required to scan and email a copy of NRIC / Passport / Visa & Proof of Residential Address (eg. Utilities bills, bank statements).
• Malaysia Companies Act requires minimum one (1) Director and one (1) Shareholder during incorporation. To register a wholly-owned subsidiary, you need to first register under individual shareholder and subsequently transfer shares to the holding company

We hereby authorise Apex Gateway Accounting. and its member firms to make the relevant filing to Suruhanjaya Syarikat Malaysia (SSM) on behalf of the Company for registration of the Company (Sdn Bhd). The Company and the Company's Shareholders & Directors also hereby fully indemnifies and holds Apex Gateway Accounting. and its member firms harmless from any loss, cost or damages in respect of inaccuracy or incomplete information filed through the service howsoever arising and fully indemnified and holds Apex Gateway Accounting and its member firms harmless from and against all claims or liabilities from any party concerned.

By completing this form, you agree that Apex Gateway will manage your personal data in accordance with the Malaysian Personal Data Protection Act 2010 (Act 709) (“The Act”) and our data protection policy.
